Kalpana Vishnupad, MD – Medical Director

Kalpana Vishnupad, MD, is a highly experienced Psychiatrist who treats adult and geriatric patients at Haven Behavioral Hospital of Dayton.  She received her medical degree from Karnatak University in Hubli, India, a psychiatry residency at Temple University Hospital in Philadelphia, and a geriatric fellowship at the Hospital of the University of Pennsylvania in Philadelphia.

In addition to her work at Haven, Vishnupad provides consultation at Springfield Regional Medical Center, and treats patients in several nursing homes and geriatric clinics.  She has been the Associate Medical Director for the Mental Health Services for Clark County, Inc., since 2008.

With her extensive experience in psychiatry, and most especially with geriatric psychiatry, Vishnupad is an invaluable resource and caregiver for those patients at Haven.

Board Certified in general and geriatric psychiatry, Vishnupad is a member of the American Geriatric Association.  In addition to treating patients, Vishnupad’s work has appeared in many psychiatric publications; she has completed psychiatric research projects, is a professional speaker on geriatric psychiatry, and is a former medical student instructor. 

Jonathan Duckett – Chief Executive Officer

Jonathan Duckett is a graduate of the University of Tennessee with a Bachelor of Arts degree in Sociology.  Duckett began his career as a Behavioral Health Technician at the Village, a long-term residential treatment facility for teenagers with co-occurring diagnoses.  Duckett was most recently the interim CEO and the Director of Risk Management and Performance Improvement at Haven.  Duckett was formerly the Director of Risk Management and Performance Improvement for Village Behavioral Health where he held positions as the Milieu Supervisor and Case Manager/Discharge Coordinator. Past experience also includes work as a clinical supervisor and counselor for Youth Villages, a non-profit that provided intensive in-home counseling to at-risk children and adolescents. 

Carrie A. Snelling, RPH, CDP – Director of Pharmacy

Carrie Snelling joined the Haven team in 2015. She has demonstrated versatility in adapting to ever-increasing complexity in the healthcare industry.  While committed to providing top-quality pharmaceutical care, Snelling delivers patient-focused pharmacy operations and services.

Snelling graduated from Ohio Northern University in 2002 with a BS in Pharmacy. Prior to her time at Haven, Snelling spent nine years in the specialized Home Infusion Pharmacy industry. Most recently, she was the Pharmacy Manager for Walgreens Infusion Services where she managed intake, pharmacy, warehouse, and operations.  Other experience includes Middletown Regional Medical Center/Atrium Medical Center.

Snelling obtained her Certified Dementia Practitioner designation in 2016. She serves as a preceptor for IPPE and APPE pharmacy students from Cedarville University.  She is currently licensed in Ohio, Pennsylvania and South Carolina. 

Philip Ofori-Yentumi, RN – Director of Nursing

Philip Ofori-Yentumi, RN, received his Associate’s degree in nursing from Galen College of Nursing, his bachelor’s degree in nursing from Miami University in Oxford and is currently pursuing a psychiatric mental health nurse practitioner license from Walden University.

Most recently, Ofori-Yentumi was the RN Supervisor for Blueridge Vista in Cincinnati where he was responsible for the oversight of the 42-bed geriatric and adult psychiatric hospital.  In addition, he assisted the Director of Nursing in staffing and managing the unit on a daily basis.  Other previous experience includes an RN Nigh Shift Supervisor at Beckett Springs Hospital in West Chester, an RN Nigh Shift Supervisor at Scarlet Oaks and an STNA (state-tested nursing assistant) at Heartland of Woodridge in Fairfield.

Ofori-Yentumi’s goal is to “enhance the lives of others” through his new position as Director of Nursing at Haven. 

Jodi N. Jackson – Director of Business Development

With over 20 years of healthcare and marketing experience, Jodi N. Jackson has led business teams to meet and exceed sales and retention expectations.  Most recently, Jackson was the Vice President of Sales and Marketing for East Ambulance where she was responsible for educating the community on private ambulance services, establishing and implementing marketing campaigns and developing an outside sales team. In addition, she was responsible for team work that resulted in a 200 percent increase of secured agreements with a 96 percent retention rate.

Jackson is also the sole proprietor of Comprehensive Life Strategies, a training and development company that specializes in tailored leadership, team building, personal/professional coaching and internal/external customer service programs.  She is a certified Behavioral Analyst, Business Coach and Public Speaker. 

Kelly Waggy – Director of Assessment and Utilization

Kelly Waggy received a Bachelor of Arts degree in Psychology from Gannon University and a Master of Arts degree in Clinical Psychology from the University of Dayton.  She began her career at Haven in the Intake Department before being promoted to the Utilization Review Manager.

Prior to Haven, she worked as a mental health technician in inpatient psychology, and then as a Crisis Support Specialist and advocate for domestic violence victims at the YWCA. Waggy was also a permanent supportive housing manager at Places, helping provide stable housing for individuals with chronic mental illness.

Lisa Shemo – Business Office Director

Lisa Shemo has been with Haven since April 2017 serving as the Business Office Manager before being promoted to Director.   Prior to Haven, she worked for Good Samaritan Hospital and Mercy Health Systems where she held several positions in Customer Service, Hospital Billing, Collections, Financial Counselor, Principal Trainer and Consulting as a Revenue Cycle Liaison all while building and maintaining collaborative relationships. Shemo holds several EPIC Software Certifications specializing in Revenue Cycle and Hospital Billing applications related to System Build, Training and Implementation.  Shemo is currently pursuing her bachelor’s degree in Business Administration at Indiana Wesleyan University.

Nichole Bumpus – Director of Human Resources

Nichole Bumpus, is an HR leader with more than 20 years of experience. She has lead HR departments in healthcare, business, for profit and not-for-profit.  Former employers include: The Columbus Zoo and Aquarium, PriMed Physicians Group, Standard Register, and Graceworks Lutheran Services.

Bumpus is a graduate of Wilberforce University and holds both a PHR and SHRM-CP certifications in Human Resources. 

Pamela Hensley, RHIA – HIM Manager/Privacy Officer

Pamela Hensley, RHIA, HIM Manager and Privacy Officer, is a Magna Cum Laude graduate from DeVry University and a Summa Cum Laude graduate from the University of Cincinnati with a Bachelor of Science in Health Sciences degree majoring in Health Information Management Administration.  She is an active member of the American Health Information Management Association (AHIMA) and has served on a national level as a member of AHIMA’s Engage Advisory Committee including the New Graduate Leadership Committee.  Currently, she serves on the HIM Advisory Board of American National University.  Hensley has over 30 years of healthcare experience having worked in both inpatient and outpatient settings.  She is passionate about providing the very best healthcare possible to all patients through ensuring health information is properly documented, kept private and secure, and only shared as HIPAA allows while at the same time protecting each patient’s medical identity.   

Courtney Cordner, MS – Director of Performance Improvement and Risk Management

Courtney Cordner began her career in the behavioral health field in 2011 as the Mental Health Technician for the Boys and Girls Home of Alaska in Fairbanks.  Most recently, Cordner was the Director of Risk Management and Performance Improvement at Garfield Park Hospital in Chicago.  Past experience also includes work as a compliance specialist for Hegira Programs Inc., a non-profit that offers mental health and substance use treatment programs.  Her passion is providing a safe environment for patients to heal.  Cordner holds a Master’s degree in Administration and a Bachelor of Science degree in Psychology, both from Central Michigan University. 

Tara Frizell, RN – Director of Nursing

Tara Frizell earned her Associate of Applied (AAS) Science Nursing from Edison State Community College. She comes to Haven with nearly 20 years of experience overseeing nursing and caring for patients in behavioral healthcare.

Frizell’s most recent role was as an RN Unit Manager for Spring Creek Rehabilitation and Nursing Center where she oversaw the daily activities and insurance pre-certifications, as well as quality assurance audits and reports. As a behavioral health nurse liaison with Wilson Memorial Hospital, Frizell managed pre-screening and referrals with referral resources and families and marketing efforts to those referral resources. In addition, Frizell’s past experience includes unit manager and charge nurse. 

Rev. Dr. Ronald Lee Tyson – Nurse Practitioner

Rev. Dr. Ronald Lee Tyson, DNP, DMin, CNP, PMHNP-BC, ANP-BC, CARN-AP, is an Advanced Practice Nurse holding dual doctorates in Psychiatric Nursing Practice and Theology and Spiritual Formation.  He is board certified as a Psychiatric Mental Health Nurse Practitioner, Adult Nurse Practitioner and Certified Addiction Registered Nurse.

Before entering the mental health industry, Tyson served as a minister and still serves as an adjunct clergy.  He earned degrees from Anderson University, Anderson University School of Theology, Knox Theological Seminary, University of Cincinnati and the University of Louisville.  Tyson completed his residency as a psychiatric provider and psych-pharmacologist at Cincinnati Children’s Hospital Medical Center. 

Carolyn Wright – Director of Social Services

Carolyn Wright received a Bachelor of Social Work degree from the University of Hawaii at Manoa and a Master of Social Work degree from The Ohio State University.  She obtained her independent licensure in 1999 and has a breadth of experience across the social service spectrum, working for a variety of community agencies throughout her 25 year career.  Wright comes to Haven from Equitas Health, where she worked as a clinical supervisor.  Prior to that position, Wright worked as a family therapist at the Dayton VA Medical Center.  In addition to her professional work, Wright serves as a Township Trustee for Bethel Township, Miami County.
